When conducting your annual progress report to the NIH you must present the number of women and minority participants in your study. This is done by filling out the Cumulative Inclusion Enrollment Report showcased below.

When collecting data on ethnicity and race, as well as sex/gender, use the categories listed to obtain the data from individuals on the basis of self-identification. The participants will themselves identify their race and ethnicity.

It is recommended to collect this information through two separate questions. Ethnicity first, followed by race, with the option to select more than one racial designation.
